The pilot's spatial disorientation and in-flight loss of control of the helicopter after encountering night instrument meteorological conditions resulting in collision with water during the resultant uncontrolled descent. A factor in the accident was the pilot's decision to attempt the flight when instrument meteorological conditions were forecasted for the area.
The pilot's failure to verify the presence of wires near the remote landing site during the high reconnaissance, and his misjudgement of the clearance from the ground during the attempted hovering turn to avoid the wire obstacle. Factors were the dark night and the incorrect information provided by the ground crew.
These match the airframe or engine model. Whether each one applies, and whether it's been complied with, depends on serial numbers, equipment and logbook entries. Applicability must be confirmed by a mechanic against the aircraft's records.
