FAILURE OF THE AN TEE FITTING, THEREBY ALLOWING THE OIL TO BE PUMPED OVERBOARD CAUSING THE ENGINE FAILURE. THE FAILURE OF THE TEE FITTING WAS CAUSED BY THE TIE WRAP USED TO SECURE THE LINE.
